Remember that direct activation of the case, the check on the correctness of the information shown in acceptance forms where set out and notification of membership of any funds or insurances is the patient’s responsibility. The patient is invited to advise membership of one of the funds indicated at the time of booking by telephone or web/mail. Direct acceptance cannot be activated at the same time as the check-up/specialist service.
The relative cards (AIK, CASAGIT and CRAL Zurich) must be shown to access the economic facilitations set out by the above agreements.
